Full-Time RN On-Call / Tuck-In Nurse
LifeSource Hospice is seeking a compassionate, experienced, and dependable Registered Nurse (RN) to provide after-hours hospice support and evening tuck-in visits for patients throughout our service area. This role is responsible for responding to urgent patient needs, providing symptom management, conducting nursing assessments, supporting families and caregivers, and ensuring patients are comfortable and safe overnight.
The ideal candidate is clinically strong, calm under pressure, highly responsive, and passionate about providing exceptional end-of-life care.
Respond promptly to hospice patient calls and urgent nursing needs after regular business hours
Perform tuck-in visits to assess patient comfort, medication effectiveness, safety, and caregiver needs
Provide skilled nursing assessments and interventions related to pain and symptom management
Educate patients and families regarding disease progression, medications, and end-of-life care
Collaborate with the interdisciplinary hospice team to ensure continuity of care
Complete timely and accurate EMR documentation in accordance with Medicare, CHAP, and agency standards
Communicate changes in patient condition to physicians and hospice leadership as appropriate
Assist with crisis management, pronouncements, and admissions when needed
Ensure compliance with hospice policies, infection control standards, and regulatory requirements
Participate in team meetings, education, and quality improvement initiatives as requested
Current Registered Nurse (RN) license in the State of New Jersey
Minimum of 5 years nursing experience required
Hospice, home health, oncology, med-surg, or critical care experience preferred
Strong assessment and clinical decision-making skills
Excellent communication and compassionate bedside manner
Ability to work independently in the field
Reliable transportation and valid driver’s license required
Current CPR certification required
Experience with hospice EMR systems (WellSky/Consolo preferred)
Knowledge of hospice Medicare Conditions of Participation and end-of-life care standards
Comfortable providing education and emotional support to patients and families during difficult situations
