SUMMARY
Cicero Family Services is seeking a compassionate, bilingual Community Outreach Worker to join our team. This dynamic, dual-focus role blends hands-on clinical case management — including completing mental health assessments and supporting clients through psychiatric appointments — with community outreach and education that raises awareness of mental health resources across Belmont Cragin and surrounding communities. ESSENTIAL DUTIES AND RESPONSIBILITIES
- Complete mental health assessments and support clients through psychiatric and medication monitoring appointments.
- Provide case management services, connecting clients with mental health services, housing, income entitlements, and community/family support.
- Conduct bilingual (English/Spanish) community outreach and education on mental health topics, with a focus on awareness, prevention, and reaching high-risk populations.
- Collect, track, and report program and outcome data accurately and in a timely manner, in accordance with agency, grant, and funder reporting requirements.
- Collaborate closely with the clinical team, supervisors, and fellow outreach staff.
What We're Looking For
- Bachelor's degree in Social Work or a Human Service field required.
- Fully bilingual in English and Spanish.
- Strong interpersonal, public speaking, and written communication skills.
- A self-starter who thrives working both independently and as part of a team.
- Comfortable working across clinical and community settings, with reliable transportation to travel within Belmont Cragin community and surrounding areas.
- Cultural humility and an understanding of culturally responsive practices
