Job Title: Per Diem Registered Nurse (RN) – Assessment Nurse
Department: Clinical Services
Reports To: Director of Patient Services (DPS)
Position Summary
The Per Diem Registered Nurse (RN) – Assessment Nurse is responsible for conducting initial and periodic nursing assessments for individuals receiving non-skilled home care services. The RN develops and updates individualized Plans of Care based on the client's needs and physician's orders, ensuring compliance with New York State Department of Health (NYSDOH) regulations and agency policies.
Essential Duties and Responsibilities
- Conduct initial nursing assessments for prospective clients receiving personal care and home health aide services.
- Complete reassessments and supervisory assessments as required by agency policy and NYSDOH regulations.
- Develop, implement, and update individualized Plans of Care based on the client's physical, functional, psychosocial, and environmental needs.
- Review physician orders and ensure the Plan of Care accurately reflects authorized services.
- Determine the appropriate level of non-skilled home care services based on the client's condition and functional status.
- Coordinate with the Director of Patient Services, scheduling staff, and care team regarding assessment findings and service recommendations.
- Identify changes in a client's condition that may require physician notification or referral for skilled nursing or other healthcare services.
- Complete all assessment documentation accurately, thoroughly, and within required timeframes using the agency's electronic medical record (EMR).
- Participate in case conferences and interdisciplinary discussions as needed.
- Maintain compliance with all federal, state, and agency regulations, including HIPAA requirements.
- Participate in required orientation, in-service education, and competency training.
- Perform other assessment-related duties as assigned.
Qualifications
- Current, unrestricted Registered Nurse (RN) license in the State of New York.
- Graduate of an accredited school of nursing.
- Minimum of one (1) year of nursing experience; home care or community health experience preferred.
- Current BLS certification preferred.
- Valid driver's license, reliable transportation, and automobile insurance if travel is required.
- Ability to travel throughout the agency's service area.
Knowledge, Skills & Abilities
- Knowledge of New York State Licensed Home Care Services Agency (LHCSA) regulations.
- Strong clinical assessment and critical thinking skills.
- Excellent documentation and organizational skills.
- Ability to work independently and manage multiple assessments.
- Strong verbal and written communication skills.
- Proficiency with electronic medical records (EMR) systems.
Physical Requirements
- Ability to travel to client residences.
- Ability to perform nursing assessments in various home environments.
- Frequent standing, walking, bending, reaching, and sitting.
Work Schedule
- Per Diem / As Needed
- Flexible scheduling based on agency referrals and assessment needs.
