We are seeking a compassionate and dedicated Licensed Social Worker to join our dynamic healthcare team. Responsibilities
- Conduct thorough patient assessments to identify social, emotional, and environmental needs, including diagnostic evaluations and mental health counseling.
- Develop individualized care plans aligned with social work practice standards and clinical ethical guidelines, incorporating behavioral therapy, motivational interviewing, and crisis intervention techniques.
- Provide direct client services such as individual and group therapy sessions, family counseling (including child & family counseling), and addiction counseling.
- Coordinate community resources to support patients’ discharge planning, home & community care, hospice & palliative medicine needs, and assisted living arrangements.
- Maintain accurate medical documentation and records in compliance with HIPAA regulations, ICD coding standards (ICD-9/10), CPT coding, and EMR systems.
- Collaborate with interdisciplinary teams including healthcare providers, nurses, therapists, and case managers to ensure holistic patient care.
- Participate in clinical research initiatives related to behavioral health or geriatrics to advance best practices and improve patient outcomes.
Requirements
- Valid state licensure as a Licensed Clinical Social Worker (LCSW) or equivalent credential.
- Proven experience working with diverse populations across settings such as hospitals, nursing homes, outpatient clinics, or home & community care environments.
- Strong knowledge of medical terminology, Medicare regulations, ICD coding (ICD-9/10), and clinical documentation standards.
- Demonstrated expertise in behavioral health modalities including cognitive behavioral therapy (CBT), trauma-informed care (PTSD), dementia & Alzheimer’s care, autism spectrum disorder interventions, and end-of-life care.
- Excellent client communication skills with the ability to conduct intake assessments and provide empathetic support during crisis management situations.
- Familiarity with EMR systems and experience working with individuals from diverse cultural backgrounds.
- Ability to handle acute care situations effectively while maintaining professionalism under pressure.
- Additional skills such as hospice & palliative medicine knowledge, post-acute care experience, inpatient experience, or working with geriatric or pediatric populations are highly valued.

Pay: $30.55 - $44.01 per hour
