Position Summary:
Unique Hospice and Palliative Care is currently seeking an experienced LVN or RN for the role of On-Call Nurse / Runner to join our dedicated and growing team. This position provides crucial care outside of regular hours, ensuring our patients receive timely support and symptom management during evenings, nights, and weekends.
Key Responsibilities:
- Promptly respond to on-call needs, offering either in-person visits or telephonic support as required.
- Conduct clinical assessments, manage urgent symptoms, and adjust patient care plans based on physician instructions.
- Act as a compassionate point of contact for patients and families, providing emotional support and education.
- Work closely with the care team to communicate patient status and follow-up actions to ensure seamless care.
Qualifications:
- Licensed Vocational Nurse (LVN) or Registered Nurse (RN) with a valid license.
- Preferred 1+ year of hospice or palliative care experience.
- Strong clinical assessment, decision-making, and communication skills.
- Ability to work independently, demonstrate flexibility, and provide support under time-sensitive conditions.
