We are seeking a dedicated and compassionate Nurse Case Manager (RN) to join our growing home care team. This full-time Nurse Case Manager (RN) position focuses on completing client assessments and reassessments to ensure high-quality, individualized care for patients in the Ashburn and Northern Virginia communities. Schedule:
Tuesday through Saturday
Shift options: 10:00 AM – 6:00 PM or 11:00 AM – 7:00 PM
Key Responsibilities:
- Conduct initial client assessments and routine reassessments in home settings
- Conduct hospital discharge assessments
- Develop and update individualized care plans
- Collaborate with caregivers, families, and interdisciplinary teams
- Monitor client progress and ensure compliance with care standards
- Maintain accurate and timely documentation
- Completing annual skills assessments and competencies for LPNs and CNAs/Caregivers,
- Training and supervising healthcare personnel: LPNs, CNAs, and Caregivers.
- Attend dischange planning as needed
Qualifications:
- Active Virginia RN license in good standing (Compact/Multi-state)
- At least two years of clinical experience (home care, case management, or patient assessment)
- Strong clinical judgment and communication skills
- Ability to work independently and manage a flexible schedule
- Current CPR Certification, AHA BLS
