Job Overview
We are seeking a dedicated and compassionate Case Manager to join our team. The ideal candidate will play a crucial role in supporting individuals and families by providing guidance, resources, and advocacy to help them navigate various challenges. This position requires a strong understanding of behavioral health, social work principles, and crisis management techniques. The Case Manager will work closely with clients to develop personalized plans that promote their well-being and empower them to achieve their goals.
Responsibilities
- Conduct comprehensive assessments to identify client needs and strengths.
- Develop individualized service plans in collaboration with clients, focusing on achievable goals.
- Utilize motivational interviewing techniques to engage clients effectively.
- Provide ongoing support and counseling in areas such as addiction recovery, mental health, and workforce development.
- Facilitate access to community resources, including Section 8 housing assistance and behavioral health services.
- Monitor client progress and adjust service plans as necessary to ensure optimal outcomes.
- Coordinate with other professionals and agencies to provide holistic support for clients.
- Maintain accurate documentation of client interactions, progress notes, and service outcomes.
- Transport Clients in an insured vehicle
- Have a valid driver's license
- Perform other duties as assigned
Experience
- Possess a master's or bachelor's degree from an approved college or university
- A background in social work or a related field is preferred.
- Experience in behavioral health settings or addiction counseling is highly desirable.
- Familiarity with crisis management strategies and workforce development initiatives is beneficial.
- Proficiency in motivational interviewing techniques will enhance client engagement.
- Previous experience working with diverse populations is an asset.
