Job Description:
Proven Home Healthcare Agency RN Case Manager will administer medication as necessary, conduct supervisory visits for HHA. create an ongoing report on the patient's progress. Our nurse's responsibilities include working with other healthcare professionals to create treatment plans, ensuring that patients stick to treatment plans, and monitoring patients' vital signs.
Our home health nurse possesses patience as well as empathy towards our clients and their families.
Responsibilities:
· Develop care plans for each new client and reassements visits as needed.
Administering medication to clients as assigned / as needed.
- Attending regular operation meetings and assit with strategic planning.
- Working with other professionals to create treatment plans.
- Cleaning and re-dressing wounds if needed.
- Monitor vital signs as needed.
- Monitoring changes in behavior and progress from caregivers' reports on portal and visits.
- Provide guidance and support to other Caregivers.
Requirements:
·At least 1 year experience in Home Health Care.
. At a minimum, an associate degree in nursing (ADN), or a Bachelor of Science degree.
· Pass the NCLEX exam.
· Patience.
· Empathy.
· Flexible work hours.
· Ability to work under pressure.
