Job Summary
The Director of Nursing is responsible for the overall clinical management of the agency, including supervision of all clinical staff, oversight of patient care, regulatory compliance, and leadership of the agency's Quality Assurance and Performance Improvement (QAPI) program. This position works closely with the Administrator to ensure compliance with Medicare Conditions of Participation, Massachusetts Department of Public Health regulations, and agency policies.
Essential Responsibilities
Clinical Leadership
- Direct and supervise all clinical operations of the home health agency.
- Oversee Registered Nurses, Licensed Practical Nurses, therapists, medical social workers, and home health aides.
- Ensure patients receive safe, high-quality, evidence-based care.
- Review and approve clinical policies, procedures, and standards of practice.
- Serve as the agency's clinical resource for staff and providers.
- Participate in strategic planning and agency leadership meetings.
Quality Assurance & Performance Improvement (QAPI)
- Lead the agency's QAPI program in accordance with Medicare Conditions of Participation.
- Monitor quality indicators, clinical outcomes, OASIS accuracy, infection prevention, hospitalization rates, and patient satisfaction.
- Conduct routine clinical record audits to ensure documentation accuracy and regulatory compliance.
- Identify trends and implement corrective action plans to improve quality outcomes.
- Coordinate Performance Improvement Projects (PIPs).
- Prepare quality reports for leadership and governing body meetings.
- Maintain continuous survey readiness and lead preparation for state and federal inspections.
Regulatory Compliance
- Ensure compliance with Medicare, Medicaid, HIPAA, OSHA, and Massachusetts home health regulations.
- Maintain agency compliance with CMS Conditions of Participation.
- Oversee OASIS documentation accuracy and timeliness.
- Collaborate with billing staff to ensure documentation supports reimbursement.
- Investigate clinical incidents, complaints, and adverse events.
- Develop corrective action plans when necessary.
Staff Management
- Recruit, interview, hire, orient, and retain qualified clinical staff.
- Provide ongoing coaching, education, and performance evaluations.
- Ensure clinical competency and annual skills validation.
- Promote employee engagement and professional development.
Patient Care Oversight
- Review admissions and clinical appropriateness of referrals.
- Ensure individualized plans of care are developed and followed.
- Coordinate interdisciplinary case conferences.
- Monitor continuity of care and patient outcomes.
- Collaborate with physicians and referral sources regarding patient needs.
Qualifications
Required
- Active Registered Nurse (RN) license in Massachusetts in good standing.
- Minimum of 1 year of leadership experience in a Medicare-certified home health agency.
- Strong knowledge of Medicare Conditions of Participation (CoPs), OASIS, PDGM reimbursement, and CMS regulations.
- Experience managing Quality Assurance and Performance Improvement (QAPI) programs.
- Experience leading successful regulatory surveys and maintaining survey readiness.
- Excellent leadership, organizational, communication, and problem-solving skills.
- Proficiency with electronic medical record (EMR) systems and Microsoft Office.
Preferred
- Certified in Healthcare Quality (CPHQ) or other quality certification.
- Experience with ACHC, CHAP or Joint Commission accreditation.
- Experience with Medicare and Medicaid billing documentation requirements.
- Knowledge of value-based purchasing initiatives and quality metrics.

Pay: $80,000.00 - $90,000.00 per year
