Job Overview
We are seeking a dedicated and skilled RN or LPN to join our team for the 7A-3P Monday through Friday shift. This vital role involves delivering exceptional patient care, performing assessments, administering medications, and collaborating with interdisciplinary teams to ensure the well-being of our residents. The ideal candidate will possess strong clinical skills, excellent communication abilities, and a passion for improving patient outcomes in a long-term care setting.
Responsibilities
- Conduct comprehensive patient assessments and develop individualized care plans aligned with best practices and regulatory standards.
- Administer medications and treatments accurately, including injections, IV infusions, and wound care such as suturing when necessary.
- Monitor vital signs, manage acute pain, and perform physical examinations to evaluate patient status effectively.
- Document all patient interactions thoroughly using electronic health record (EHR) systems such as Cerner or Athenahealth while maintaining HIPAA compliance.
- Collaborate with physicians, therapists, and other healthcare professionals to coordinate care plans and ensure continuity of care across settings.
- Assist with diagnostic evaluations including laboratory specimen collection and processing, as well as basic imaging procedures like CT scans when applicable.
- Provide education and support to residents and their families on health management, medication administration, and discharge planning.
- Maintain aseptic techniques during procedures such as catheter care, phlebotomy, and sterile dressing changes to prevent infections.
Requirements
- Valid RN or LPN license in good standing with relevant state licensing boards.
- Prior experience in long-term care facilities
- Strong assessment skills with the ability to recognize changes in patient condition promptly.
- Excellent communication skills combined with a compassionate approach to patient care.
