What You'll Be Doing
As a Floating Visiting Nurse, you'll provide skilled nursing visits throughout the Twin Cities while helping patients safely manage their health at home.
Responsibilities include:
- Conducting skilled nursing visits and comprehensive assessments.
- Providing wound care, medication management, and patient education.
- Developing and updating individualized plans of care.
- Supporting patients and families with disease management and safety.
- Assisting nearby territories based on patient and operational needs.
- Collaborating with therapists, physicians, and interdisciplinary team members.
- Completing documentation and OASIS assessments when applicable.
- Participating in limited on-call responsibilities.
Qualifications
Active Minnesota Registered Nurse License Required
- Strong clinical and communication skills.
- Ability to work independently in a field-based environment.
- Home health experience preferred but not required.
- Valid driver's license and reliable transportation.
