Under the supervision of the Director of Social Services, the Community Health Coordinator supports individuals who are at increased risk of HIV infection or who face barriers to accessing HIV prevention, STI treatment, and medical care. The Community Health Coordinator serves as a trusted liaison between the community and healthcare providers, helping clients access PrEP services, connect with essential resources, navigate the healthcare system, and remain engaged in ongoing care. This role focuses on outreach, health education, advocacy, and collaboration to improve health outcomes and reduce disparities within the community.

Conduct outreach to identify individuals at risk for HIV and those who have been lost to care.

Collaborate with Unity providers, nursing staff, social services, and community partners to reconnect patients with medical care.

Educate clients on HIV prevention, PrEP, STI prevention and treatment, and available healthcare services.

Link clients to Community Health Navigators, Health Coaches, and other support services for PrEP and ongoing care.

Develop trusting, therapeutic relationships with clients and provide individualized support throughout their care journey.

Assist clients in overcoming barriers related to housing, substance use, mental health, domestic violence, incarceration, pregnancy, and other social determinants of health.

Encourage patient engagement and adherence to PrEP treatment and preventive healthcare services.

Document patient encounters, referrals, and follow-up activities accurately in eClinicalWorks (eCW) within established timelines.

Participate in professional development, departmental meetings, and required trainings.

Perform other duties as assigned.

Bachelor's degree in a health-related field, social services, public health, or a related discipline.

Experience working with individuals affected by HIV, substance use disorders, homelessness, incarceration, or other vulnerable populations.

Experience in community health, case management, social services, or public health preferred.

Knowledge of HIV prevention, PrEP, STI prevention, and community resources is preferred.

Strong verbal and written communication, interpersonal, and organizational skills.

Ability to build trusting relationships with diverse populations and work both independently and collaboratively.

Proficiency with Microsoft Office applications and electronic health records is preferred.
