EXAMPLE OF DUTIES
This role focuses on assessing needs, initiating and adjusting care plans, and helping consumers navigate and access the supportive services they need. Work encompasses intake, assessments, referrals, ongoing care management, caregiver support, and collaborating with other professionals and providers to maintain safety and quality of life for the seniors and families we serve. As a Aging Care Manager 2, you will perform the following duties: Case Screening and Intake: Handle telephone intake, screen assigned cases within required timelines, and gather information needed to prioritize cases and determine eligibilityField Assessments: Make home and facility visits to gather more information about each individual requesting services by using the Department of Aging standardized assessment tool to determine level of careTimely Documentation: Accurately enter and update records in our electronic system within required timeframesService Navigation: Maintain knowledge of AAA programs and community supports to guide consumers and offer informed service choicesCare Planning and Reassessment: Develop, initiate, and adjust care plans, cost share calculations, and level of care in line with regulations and agency policyFinancial and Crisis Support: Arrange financial management when needed, monitor services, respond to emergencies, and resolve problems to support consumer stabilityWork Schedule and Additional Information: Full-time employmentWork hours are 8:00 AM to 4:00 PM, Monday - Friday, with a 30-minute lunch.Telework: You will not have the option to telework in this position.
