Are you passionate about helping seniors access the care they need? Northland PACE Senior Care Services is looking for a dedicated and detail-oriented Intake/Enrollment Coordinator to guide seniors and families through the enrollment process and promote our life-changing program!
What You’ll Do:
- Conduct in-home, nursing home, and hospital assessments
- Maintain detailed records and determine funding sources (Medicaid, Medicare, private pay)
- Engage in marketing & outreach to connect with the community

Must have:
- Minimum education, Associates Degree. Experience may substitute for educational requirement
- Minimum of two years direct service experience in a healthcare setting
- Proof of a valid driver’s license, good driving record and auto insurance as required by state law
