Job Overview
We are seeking a dedicated and highly skilled Wound Care Nurse Practitioner to join our multidisciplinary healthcare team. The ideal candidate will possess extensive clinical expertise in wound management, advanced assessment skills, and a passion for improving patient outcomes. As a Wound Care Nurse Practitioner, you will provide comprehensive wound assessment, develop individualized treatment plans, and collaborate with physicians and other healthcare professionals to deliver exceptional patient care across diverse settings. Responsibilities
- Conduct thorough wound assessments, including evaluating wound size, depth, exudate, and signs of infection using advanced techniques such as sonography or sterile processing when applicable.
- Develop and implement evidence-based treatment plans tailored to each patient's needs, incorporating therapies such as debridement, dressings, compression therapy, and botulinum toxin treatment for specific conditions.
- Perform specialized procedures including suturing, catheterization, IV infusion, and sterile technique management to promote optimal healing.
- Collaborate closely with physicians, physical therapists, dietitians, and case managers to coordinate comprehensive care plans for patients in outpatient clinics, nursing homes, assisted living facilities, or post-acute care settings.
- Educate patients and their families on wound care protocols, infection prevention strategies, and lifestyle modifications to support healing and prevent recurrence.
- Maintain accurate medical documentation within EMR systems like Epic or eClinicalWorks while ensuring compliance with HIPAA regulations.
- Participate in clinical research initiatives related to wound management innovations or new therapies such as dermal fillers or esthetic laser treatments.
- Stay current with advancements in critical care, hospital medicine, infectious disease care, and trauma center protocols to provide the highest standard of wound care expertise.
Qualifications
- Current Nurse Practitioner licensure with certification in wound management or related specialties preferred.
- Demonstrated experience in acute care environments such as ICU or Level I Trauma Centers is highly desirable.
- Proficiency with EMR/EHR systems including Epic, Cerner, or Athenahealth for documentation and patient tracking.
- Strong clinical skills in performing procedures such as suturing, phlebotomy, venipuncture, catheter care, and sterile processing techniques.
- Knowledge of CPT coding and ICD coding systems for accurate documentation and billing purposes.
- Experience working with diverse patient populations including pediatrics, geriatrics, hospice & palliative medicine patients, and individuals with developmental disabilities.
- Familiarity with telehealth platforms to support remote patient assessments when necessary.
- Excellent communication skills combined with critical thinking abilities to triage urgent cases effectively and manage complex cases involving infectious disease or acute pain management.
