Job Summary
We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will provide high-quality nursing care to patients in various settings, including hospitals, clinics, and long-term care facilities. As an LPN, you will work under the supervision of registered nurses and physicians, assisting in patient assessments, administering medications, and ensuring the comfort and well-being of patients.
Duties
- Provide direct patient care including monitoring vital signs and assisting with daily living activities.
- Administer medications and treatments as prescribed by healthcare providers.
- Conduct physical examinations and document patient health histories.
- Assist in the management of patients with specific conditions such as dementia and those requiring infant care.
- Support airway management procedures as necessary.
- Participate in sterile processing of medical instruments to ensure a safe environment for patient care.
- Collaborate with healthcare team members to develop and implement patient care plans.
- Educate patients and their families about health conditions, treatment plans, and preventive measures.
- Maintain accurate records of patient care activities in compliance with ICD-10 coding standards.
- Documentation with EMR of PointClickCare.
Skills
The successful candidate should possess the following skills:
- Proficiency in appropriate practices and procedures.
- Strong understanding of Dementia care techniques.
- Knowledge of SNF settings
- Experience in LTC/ SNF medicine settings.
- Familiarity with care practices.
- Ability to perform Physical examinations effectively.
- Proficient in EMR documentation
- Competence in processing techniques.
- Familiarity with ICD-10 coding for accurate documentation.
