Duties
- Assist individuals with daily living activities such as personal hygiene, meal preparation, and mobility support
- Able to manage active DSP's
- communicate with program manager directly
- Monitor health status and report changes using Electronic Health Records (EHR) systems with accuracy and confidentiality
- Implement care plans tailored to each individual’s needs, including medication reminders and behavioral support strategies
- Provide critical care support in emergency situations, utilizing Basic Life Support (BLS) and CPR techniques when necessary
- Support clients during medical appointments or diagnostic procedures, including procedures related to surgery or radiology
- Maintain a safe environment through manual handling techniques and adherence to safety protocols
- Foster positive relationships by engaging clients in recreational activities, skill-building exercises, and community outings
Job Summary
Provide compassionate, person-centered support to individuals with intellectual and developmental disabilities (IDD) and other disabilities while promoting independence, dignity, inclusion, health, and safety.
- Drive clients to medical appointments and community outings (parks, recreation, religious services, shopping, etc.)
- Provide personal care including bathing, grooming, hygiene, incontinence care, mobility assistance, adaptive equipment support, meal preparation, ADLs, health and wellness support, medication reminders/assistance, employment and education support, volunteer/community participation, life skills training, social/emotional support, and assisting clients in communicating their wants and needs.
Responsibilities

- Encourage independence and achievement of client goals.
- Maintain client safety and accurate documentation.
- Report incidents and accidents promptly.
- Respect client rights and always treat individuals with dignity, kindness, and respect.
Qualifications
18+, HS diploma/GED, 1 year IDD/disability experience, CPR/First Aid or willingness, valid driver's license, clean record, reliable transportation, proof of insurance, pass background check, fingerprinting and drug screen, complete DDD/Medicaid training, lift up to 50 lbs, communication and basic computer skills. Preferred: CNA/HHA (not expiring within 90 days), medication administration experience, related college coursework.
Pay: $24.00 - $28.00 per hour
