Responsibilities
- Provide direct patient care including assistance with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using patient transfer methods and equipment like Hoyer lifts.
- Observe and monitor patients’ health status by taking vital signs, noting changes in condition, and reporting findings accurately using Electronic Health Record (EHR) systems while maintaining strict patient confidentiality in accordance with HIPAA regulations.
- Assist with medication administration under supervision, including managing catheter care, wound care, and other medical procedures following home healthcare regulations.
- Support patients with memory care needs such as Alzheimer’s disease or dementia by implementing specialized techniques to promote safety and comfort.
- Perform light housekeeping duties including cleaning, laundry, meal preparation, and ensuring a safe environment aligned with home health care standards.
- Communicate effectively with patients, families, and healthcare team members to coordinate care plans and provide emotional support during end-of-life stages.
- Use proper patient transfer methods and equipment like Hoyer lifts to ensure safe mobility while preventing injury for both patients and caregivers.
Qualifications
- Prior experience assisting with home health care or in assisted living environments is highly preferred.
- Knowledge of medical terminology, vital signs measurement, medication administration, and basic life support (BLS) or CPR certification is required.
- Experience working with geriatric patients or individuals with disabilities—including Alzheimer’s or dementia—is essential.
- Familiarity with EMR/EHR systems for documentation along with understanding of home health agency regulations is advantageous.
- Ability to lift heavy objects safely and perform physical tasks such as transferring patients or performing light cleaning is necessary.
- Strong communication skills to effectively interact with patients’ families and healthcare teams while maintaining patient confidentiality under HIPAA guidelines.
- Certification in first aid is preferred; ongoing training in hospice care practices is encouraged to enhance your expertise.
