Home health occupational therapy has a different rhythm.
Your first patient may be recovering from a joint replacement and learning how to safely navigate their bathroom.
Your next may need adaptive equipment to remain independent.
Another may be rebuilding the strength, coordination, and confidence needed to prepare meals or get dressed without assistance.
Rather than spending your entire day inside the same clinic or facility, your patients become your destination.
You'll travel within an assigned territory and provide individualized therapy in the environment where your recommendations actually need to work.

You can teach someone how to complete a transfer in a therapy gym.
But what happens when their bathtub has a high ledge?
Their bedroom is upstairs?
Their kitchen counters are difficult to reach?

Home health allows an Occupational Therapist to see the complete picture.
You're not recreating someone's home environment inside a clinic.
You're standing in it.
That gives you the opportunity to develop practical solutions based on how your patient actually lives.
What You'll Do
- Complete in-home occupational therapy evaluations.
- Develop individualized plans of care and measurable functional goals.
- Help patients improve their ability to safely complete activities of daily living.
- Address bathing, dressing, grooming, toileting, meal preparation, mobility, and other functional activities.
- Evaluate strength, coordination, fine motor skills, cognition, safety, and functional performance.
- Complete home safety assessments.
- Identify fall risks and environmental barriers.
- Recommend adaptive equipment and home modifications.
- Educate patients, caregivers, and family members.
- Help patients regain independence following illness, injury, surgery, or hospitalization.
- Collaborate with physicians, nurses, PTs, SLPs, social workers, caregivers, and other members of the care team.
- Complete accurate and timely EMR documentation.

Previous home health experience is valuable, but it isn't the only background that can prepare you to succeed.
We're looking for an OT who is clinically strong, organized, comfortable working independently, and passionate about helping patients function safely in their everyday lives.
What We're Looking For
- Current Minnesota Occupational Therapist (OTR/L) license.
- Valid driver's license and reliable transportation.
- Strong clinical assessment and treatment-planning abilities.
- Ability to manage a territory-based schedule independently.
- Excellent patient and caregiver education skills.
- Strong communication and interdisciplinary collaboration.
- EMR experience preferred.
- Commitment to providing exceptional patient-centered care.
