You'll bring rehabilitation directly to patients in the comfort of their homes, with a supportive clinical team behind you every step of the way.
Compensation — Full-Time:
Earn $58,000–$105,300 depending on your employment type and patient volume.

W2 Salary: $58,000–$75,000/year (may increase based on patient volume).

W2 Tiered: $35–$50 per visit, rate increases with weekly volume.

1099 Tiered: $50–$60 per visit, with earning potential up to $105,300/year.

What you'll do:

Deliver skilled PT treatments in patients' homes across the Monroe area.

Work under the supervision of a Physical Therapist (including remote supervision).

Build relationships with a consistent patient caseload.

Complete documentation using our streamlined process.

Coordinate with patients, families, and the broader care team.
What we're looking for:

Active New York PTA license.

Associate degree from an accredited PTA program.

Comfortable working independently in a field-based setting.

Strong communication and organizational skills.

Valid driver's license and reliable transportation.

Passion for delivering patient-centered care.
