Position Summary:
The Intake Specialist serves as the first point of contact for families and clients seeking services. This role is responsible for conducting outreach calls, gathering intake information, completing pre-assessments, managing referrals, and ensuring accurate documentation within the EMR system. The Intake Specialist plays a critical role in building positive relationships with families while supporting seamless entry into services.
Key Responsibilities:
- Client Outreach & Communication
- Contact families by phone, email, or text to encourage program sign-ups.
- Requires some in person meetings to be more personable and get wet signatures.
- Provide clear information about available services, group locations, time and next steps.
- Answer initial questions regarding the program. – share brochure with guardians
- Intake & Pre-Assessment
- Guide families through the intake form completion over the phone (MYIO portal)
- Conduct pre-assessments to collect preliminary clinical/behavioral information.
- Ensure clint documentation and information meets compliance standards. (complete file)
- Referrals & Coordination
- Process and track incoming referrals – Google sheet
- Cold calls - Follow up with referral sources to confirm receipt of brochure sent to families and answer any questions.
- Coordinate with internal site manager to assign clients to appropriate services/group.
- Data Management (EMR)
- Create and maintain accurate client profiles in the EMR system (Valant).
- Upload intake forms, assessments, and required documents as applicable
- Monitor new perspective clients – move over into patient portal
- Monitor for missing or incomplete data.
- Administrative Support
- Maintain intake and referral logs in google sheet.
- Provide weekly status reports on Team meeting call of new intakes, pending referrals, and completed assessments.
- Ensure compliance with confidentiality and HIPAA regulations.
Qualifications:
- High school diploma or equivalent
- Experience in social services, behavioral health, or customer service.
- Strong organizational and data entry skills.
- Excellent communication and active listening skills.
- Proficiency with EMR systems, Google Workspace, and spreadsheets.
Pay: From $20.00 per hour
