We are looking for a Director of Memory Care to lead our Memory Care department at The Blake at Hollingsworth Park.
This is a hands-on leadership role. You will spend time with residents and families, work alongside your team, coordinate care, manage the day-to-day operation of the department, and make sure our staff have the training and support they need to do their jobs well.

- Can have a difficult conversation when necessary while still treating the person with respect.
- Understands that accountability is part of caring for a team. You will be expected to address problems, follow up, and hold people to the standards we have established.
- Enjoys teaching and coaching others.
- Does not believe training stops after orientation. We are looking for someone who will continually educate, coach, and reinforce good practices with the team.
- Is organized, dependable, and able to manage the many moving pieces that come with leading a Memory Care department.
- Can stay calm and thoughtful when things do not go according to plan.
What you will be responsible for:
- Leading, coaching, and developing the Memory Care team.
- Providing ongoing training and education related to dementia care, resident needs, safety, communication, and community expectations.
- Making sure new employees receive the support and training they need to be successful.
- Managing the department schedule and making sure staffing needs are appropriately addressed.
- Monitoring labor, overtime, and other departmental expenses and working within the established budget.
- Overseeing hiring, onboarding, performance management, and retention of team members.
- Completing and overseeing resident assessments, service plans, and care plans.
- Monitoring changes in resident condition and making sure appropriate follow-up occurs.
- Coordinating care and communication between residents, families, physicians, nurses, pharmacies, hospice, home health, therapists, and other healthcare providers and community partners.
- Communicating with families and keeping them informed about their loved one's care and changing needs.
- Responding to family concerns and following through to resolution.
- Maintaining accurate documentation and ensuring the department follows company policies, procedures, and applicable regulations.
- Supporting the team when challenging situations arise and helping staff understand how to respond to dementia-related behaviors with patience and compassion.
- Recognizing good work and helping team members grow in their roles.
- Being willing to step in and help when the department needs you.
What we need from you:
- Current LPN or RN license required.
- At least 2 years of experience working with the geriatric population.
- Experience working with individuals living with Alzheimer's disease and related dementias.
- Previous Memory Care leadership or supervisory experience preferred.
- Experience managing staff schedules and staffing needs.
- Experience working with departmental budgets and labor.
- Experience coordinating care with families, physicians, healthcare providers, pharmacies, hospice, home health, and other community partners.
- Experience with assessments and care plans.
- Yardi and eMAR experience required.
- CPR and First Aid certification required.
- Strong communication and organizational skills.
- Ability to maintain confidentiality.
- Ability to coach, train, and hold employees accountable in a fair and respectful way.
