<Job Summary>
Join our compassionate team as a Hospice Case Manager, dedicated to providing exceptional end-of-life care to Spanish-speaking patients and their families. <Responsibilities>
- Conduct thorough patient assessments to determine eligibility and develop individualized care plans that align with patient wishes and medical needs.
- Coordinate multidisciplinary team efforts to deliver holistic hospice care, including symptom management, emotional support, and spiritual guidance.
- Communicate effectively with Spanish-speaking patients and families to explain treatment options, care procedures, and advance directives in their preferred language.
- Monitor patient progress regularly, adjusting care plans as necessary to optimize comfort and quality of life.
- Facilitate smooth discharge planning and transitions between inpatient, outpatient, or post-acute care settings.
- Maintain detailed medical records in compliance with HIPAA regulations while ensuring accurate documentation of patient interactions and interventions.
- Collaborate with healthcare providers across hospital medicine, emergency medicine, behavioral health, and primary care to coordinate services seamlessly.
<Requirements>
- Fluent in Spanish and English with excellent verbal and written communication skills to serve diverse patient populations effectively.
- Proven experience in critical care environments such as ICU, emergency room, or Level I/II trauma centers; experience in hospital medicine or acute care settings is highly valued.
- Strong background in medical-surgical nursing, geriatrics, pediatrics (PICU), or post-acute care; familiarity with infectious disease care and physiology knowledge is preferred.
- Demonstrated expertise in case management, discharge planning, utilization management, diagnostic evaluation, and patient assessment processes.
- Knowledge of medical terminology, HIPAA regulations, medical records management, vital signs monitoring, and inpatient/outpatient clinical workflows.
- Experience working with managed care organizations and understanding of healthcare utilization management principles.
- Ability to handle complex cases involving behavioral health issues or chronic illnesses with sensitivity and professionalism.
- Relevant certifications such as nursing license (RN), case management certification (CCM), or other related credentials are desirable.
