The Opportunity
We are seeking exceptional Nurse Practitioners and Physician Assistants who want to practice comprehensive primary care in a supportive, team-based environment designed to serve both patients and fellow clinicians.
You will serve as a primary clinician for a panel of patients, developing long-term relationships while leading comprehensive preventive care, chronic disease management, acute care, and care coordination.
You will be supported by a multidisciplinary care team that includes physicians, care managers, pharmacists, remote patient monitoring staff, and population health resources—allowing you to focus on practicing medicine rather than navigating healthcare alone.

What You’ll Do
- Deliver comprehensive primary care services to patients in an outpatient setting.
- Build long-term relationships with patients and families as their trusted healthcare provider.
- Manage acute illnesses, chronic diseases, preventive care, and care transitions.
- Lead chronic disease management efforts for conditions such as hypertension, diabetes, heart disease, COPD, and obesity.
- Collaborate closely with care management, pharmacy, and remote monitoring teams to support patients between visits.
- Use population health tools to identify care gaps and proactively engage patients.
- Document accurately and completely within athenahealth.
- Participate in quality improvement and value-based care initiatives.
- Help achieve meaningful clinical outcomes, including improved control rates, preventive screening completion, reduced avoidable utilization, and successful care transitions.
- Work closely with collaborating physicians and fellow APPs in a highly collaborative environment.
What We’re Looking For
Required
- Graduate of an accredited Nurse Practitioner (FNP or AGNP) or Physician Assistant program.
- Active North Carolina license (or eligibility for licensure).
- Active DEA registration or ability to obtain prior to employment.
- Strong clinical judgment and comfort managing complex adult patients.
- Commitment to relationship-based, patient-centered care.
- Excellent communication and documentation skills.
- Desire to work as part of a collaborative care team.
Preferred
- Experience in primary care, family medicine, geriatrics, or internal medicine.
- Experience with value-based care, population health, ACOs, MSSP, chronic care management, or remote patient monitoring.
- Familiarity with athenahealth.
Physical Requirements
- Ability to sit, stand, walk, bend, and reach throughout the workday.
- Ability to perform physical assessments and office-based procedures.
- Ability to communicate effectively with patients, families, and staff.
- Ability to lift and move up to 25 pounds occasionally
