Job Summary
We are seeking a dedicated and compassionate Personal Care Assistant / Homemaker to provide essential support and care to individuals in their homes and community settings. The ideal candidate will possess a strong background in home health, senior care, or assisted living environments, with a passion for enhancing clients' quality of life through personalized assistance. This role involves delivering patient-centered care, assisting with daily activities, and maintaining a safe and clean environment to promote independence and well-being.
Duties
- Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, and mobility support.
- Monitor patient health by noting changes in condition, and reporting concerns to healthcare professionals.
- Prepare nutritious meals tailored to dietary needs and assist with meal planning and preparation.
- Support medication reminders following care plans while adhering to HIPAA regulations for confidentiality.
- Maintain a clean, organized living environment by performing light housekeeping tasks such as cleaning and laundry.
- Arrange transportation assistance for appointments or community outings as needed.
- Document care activities accurately and communicate effectively with clients, families, and healthcare teams to ensure coordinated support.
Qualifications
- Experience working in group homes, assisted living facilities, nursing homes, or home health environments is helpful.
- Familiarity with care plans, HIPAA compliance, and documentation standards for patient records.
- Excellent communication skills for client interaction and team collaboration; compassionate attitude towards clients’ needs.
