Responsible for delivering care to individuals with developmental and/or intellectual disabilities by utilizing the nursing process of assessment, planning, intervention, implementation, and evaluation. Responsible for medication management, yearly nursing assessments for individuals on the caseload, and supporting the teaching to authorized direct support professionals in the residential and day program setting. The complex care management RN will effectively utilize care planning principles for individuals with complex care needs, establish and monitor health goals, and interact with the supported people, the care team, residential staff, day program staff, and health team members while maintaining all standards of professional nursing. The complex care management RN collaborates with the behavioral health department to deliver billable RN services related to medication administration education, transition planning, and overall health and wellness including, but not limited to, proper diet, exercise, and/or infection prevention education.
- Graduate of an accredited school of nursing.
- Valid registration as a Registered Nurse.
- Completion of relevant state Board of Nursing approved.
- Current AHA Healthcare Provider Training (BCLS).

Department/Program Development and Coordination (Delivery of Quality Service)
1. As a mandated reporter, ensure all allegations of abuse and neglect are reported as required.
2. Ensure compliance with all HIPAA and confidentiality requirements.
3. Complete all mandatory documentation accurately, honestly, and on time.
4. To verify and ensure your payroll timecard is accurate by Monday before noon for the previous week worked.
5. Need to complete all required training within 120 days of initial employment and stay in compliance with all the required annual training refreshers.
6. Adherence to Illinois Rule 116, provide medication authorization training to unlicensed staff following Illinois Rule 116 standards and EP!C standards. Supervision of and responsibility for nursing tasks delegated to unlicensed staff.
7. Performs yearly nursing assessments for individuals on their caseload. Assess the person to collect and document their current health status and medical history. Make accurate and ongoing assessment of their status responding and documenting appropriately.
8. Performs yearly SAMA (self-medication administration assessments) for individuals under RN’s caseload in order to assess ability for self-medicating. As needed, provides education and guidance as it relates to helping assure success for those who are SAMA independent.
9. Collaborate with other nurses and health team members to ensure the person's well-being as evidenced by feedback from the person, their family, guardians, and staff.
10. For individuals on caseload, develops comprehensive care plans and goals that are monitored and fostered for success.
11. Follows individuals on caseload to assure that primary care visits are occurring as needed, that speciality visits are scheduled (as needed), and that individuals are receiving post-hospitalization and/or post-emergency department follow-visits in order to best avoid readmissions.
12. Communicate pertinent information by phone, fax, email, and electronic programs to the appropriate people in a timely manner to include people supported, pharmacies, medical provider offices, and needed EP!C team members.
13. Collaborate with the behavioral health department to deliver services that promote wellness, proper diet, chronic disease management/prevention along with focused support services for medication management and self-medication administration (when appropriate).
14. Demonstrate accurate and timely documentation and RN case notes according to guidelines. For services that are billable through behavioral health, documents services to align with billing requirements.
15. As a RN-T (RN trainer), is actively involved with developing and delivering the supportive training and advice needed to ensure the highest level of quality for staff authorized to administer medications.
16. Works to assure that the eMAR (electronic medication administration record) system is up to date and reflective of orders that have been filled by the pharmacy. Assures that diagnosis list in the EHR/eMAR is up to date for individuals on the RN’s caseload.
17. Educates authorized direct support professionals on an individual’s medication change including assuring clarity on the type of change (medication, dose, route, etc...) along with any pertinent side effects to monitor for.
18. Educates staff, including but not limited to direct support professionals, QIDPs, team leads, operations managers, and host home providers on key care plans elements in order to assure that the interdisciplinary team is educated on the care plan and the goals.
19. Educates and assists staff and host home providers on how to use equipment that includes, but is not limited to glucometers, O2 saturation monitors, CPAPs, insulin pumps, and oxygen tanks.
20. Receive and handle phone calls during the day (regular working hours) as it relates to medication-related questions and questions relevant to individuals on the RN’s caseload.
