Duties
- Conduct thorough patient assessments, including diagnostic evaluations and mental health screenings, to identify individual needs and develop tailored care plans.
- Provide clinical counseling, including behavioral therapy, motivational interviewing, and psychotherapy, to support patients coping with a variety of mental health issues.
- Facilitate group therapy sessions and conduct crisis intervention to address urgent patient needs effectively.
- Manage case documentation accurately within electronic health records (EHR) systems while ensuring compliance with HIPAA regulations for patient confidentiality.
- Collaborate with healthcare providers, family members, and community resources to coordinate comprehensive care plans.
- Familiarity with EMR systems and CPT coding for documentation purposes; experience with ICD-10 coding is preferred.
- Strong background in crisis management, diagnostic evaluation, and case management across diverse patient populations.
