Hospice RN Case Manager
Full job description
We are seeking a compassionate and experienced Hospice Registered Nurse (RN) Case Manager to provide high-quality, patient-centered end-of-life care. The Hospice RN Case Manager is responsible for coordinating and managing the care of hospice patients and their families in accordance with the interdisciplinary plan of care and CMS Hospice Conditions of Participation.

This role requires strong clinical judgment, emotional intelligence, and a deep commitment to dignity, comfort, and holistic support within a 50 mile radius of Columbia, SC 29223.
Primary Responsibilities
- Conduct comprehensive hospice admissions and ongoing assessments
- Develop and manage individualized Plans of Care in collaboration with the Interdisciplinary Group (IDG)
- Coordinate services with physicians, social workers, chaplains, hospice aides, and therapists
- Provide skilled nursing care, symptom management, and pain control
- Educate patients and families on disease progression, medication management, and end-of-life expectations
- Ensure compliance with CMS Hospice Conditions of Participation and regulatory standards
- Participate in weekly IDG meetings and case reviews
- Complete accurate and timely documentation in the EMR
- Facilitate transitions of care, including inpatient hospice and hospital coordination
- Support families through bereavement preparation
- On-Call Rotation
Qualifications
- Active and unrestricted Registered Nurse (RN) license in South Carolina or Multi-State
- Minimum of 1–2 years of nursing experience (Hospice or Home Health preferred)
- Strong knowledge of symptom management and hospice care principles
- Excellent assessment and communication skills
- Ability to work independently in a field-based setting
- Valid driver’s license and reliable transportation
- CPR certification (required)
Preferred:
- Hospice experience
- CHPN (Certified Hospice and Palliative Nurse)
Pay: $80,000.00 - $90,000.00 per year
