Duties
- Conduct comprehensive patient assessments, including vital signs, physical examinations, and medical history documentation.
- Administer medications, injections, IV infusions, and perform suturing with precision and adherence to aseptic techniques.
- Manage complex care procedures such as ventilator support, dialysis, catheterization, tube feeding, and wound care including ostomy management.
- Utilize electronic health record (EHR) systems like Epic, Cerner, or eClinicalWorks to document medical records accurately and efficiently.
- Coordinate with multidisciplinary teams for discharge planning, case management, and long-term care strategies tailored to individual patient needs.
Requirements
- Valid nursing license with current registration in the relevant jurisdiction.
- Proven experience working in diverse healthcare environments including SNF's long-term care settings such as nursing homes or assisted living facilities.
- Proficiency with EMR/EHR systems such as Epic or Cerner; familiarity with ICD-9/10 coding and medical documentation standards.
- Strong knowledge of physiology, anatomy, pharmacology, infection control practices, HIPAA regulations, and medical terminology.
- Demonstrated ability to perform advanced procedures including IV insertion, phlebotomy, specimen collection/processing, ventilator management, and sterile processing techniques.
- Excellent patient assessment skills combined with the ability to handle acute care situations confidently.
- Experience working with individuals with disabilities or developmental delays is a plus; familiarity with geriatrics and memory/dementia care is desirable.
