Vacancy is in the Department of Health and Human Services. This is professional social services to the community through various programs in the Department. The current opening is in Maternal/Child Health whose primary function is to help assess, refer, and link prenatal and postpartum families to improve the health, safety, and overall well-being of families to improve positive birth outcomes and healthy child development. Interviews clients; collects information; assesses information; determines eligibility and may make referrals to other agencies and community support. Offers education and support to promote positive health outcomes. Maintains records and detailed information of cases; verifies information received from clients, and follow up on client's progress.
If selected, the incumbent will participate in programs to become a Certified Child Passenger Safety Technician (CPST) and a Certified Community Health Worker within 18 months.

This is a CHPEA Union position. The hours of work are 40 per week.
This is a grant-funded position renewable on an annual basis. The City of Hartford is not obligated to fund this position upon termination of funding.
