Job Summary
We are seeking a dedicated and compassionate Case Manager (QIDP - Qualified Intellectual Disabilities Professional) to support individuals within our Intermediate Care Facility (ICF) program in the Waxahachie area. The ideal candidate will possess a strong background in case management, with expertise in coordinating services for individuals with intellectual and developmental disabilities. As a vital member of our team, you will oversee person-centered planning, facilitate access to community resources, and ensure compliance with all regulatory standards. Your role will be instrumental in promoting independence, health, and well-being for the individuals served.
Responsibilities
- Conduct comprehensive assessments of residents’ needs, including medical, behavioral, and social evaluations.
- Develop individualized service plans that align with each resident’s goals and preferences, ensuring person-centered approaches.
- Facilitate communication between residents, families, healthcare providers, and team members to promote transparency and collaborative decision-making.
- Assist with discharge planning and transition processes for residents moving between inpatient, outpatient, or post-acute care settings.
- Maintain accurate medical records, documentation of assessments, care plans, and progress notes in compliance with agency policies.
- Support staff training on medical terminology, patient assessment techniques, and behavioral health best practices to enhance overall care quality.
- Ensures on-going active treatment.
- Conducts annual, new admission, and discharge staffings.
- Coordinates consultant services such as psychological, social worker, etc.
- Maintains good communications with families and guardians as needed, but at least quarterly.
- Maintains individual program plans and objectives.
- Responsible for program reviews.
- Monitors and maintains compliance with life safety code/ICF standards/ICF
- Participates in annual state/federal surveys
- Provides programmatic instruction, including necessary training of residential directors in respective facilities
- Observes all safety rules and regulations-- maintaining a healthy and safe environment at all times for the Consumers
Knowledge and Skills Required:
- Bachelor's degree from an accredited college or university required (in a social service-related field preferred) and at least one year experience working with developmentally disabled adults
- The ability to deal with concepts and complicated subject matter
- Efficient and Effective Communication skills: both written and oral
- Ability to handle multiple tasks effectively and efficiently
- Relevant certifications such as QIDP (Qualified Intellectual Disabilities Professional), along with applicable licensure or training in social work or case management fields preferred. - Valid Texas Driver’s license and good driving record
