This position is not limited to the 78216 ZIP code. Client assignments are available throughout San Antonio, surrounding communities, and the Texas Hill Country. Work locations vary based on client needs, caregiver availability, qualifications.
SCHEDULE / EXPECTED WORK HOURS:
Schedules vary based on client assignments and business needs. To be available at all times during regular business hours and as needed to provide support and ensure quality care delivery to home care clients.

We are seeking an experienced and compassionate Registered Nurse (RN) Clinical Supervisor to join our team. The Clinical Supervisor is responsible for leading and supporting our clinical team while ensuring the delivery of safe, effective, and high-quality homecare services.

Position Summary
The Clinical Supervisor manages, supervises, coordinates, evaluates, and develops client care teams to ensure quality care delivery within the scope of Agency services and in compliance with applicable policies, standards, laws, and the State Nurse Practice Act.
Reports to: Director of Clinical Services
Supervises: RNs, LPNs/LVNs, Home Health Aides, and other direct care staff
Qualifications
- Graduate of an accredited school of professional nursing; BSN preferred.
- Current, unrestricted RN license in the state(s) of practice.
- Minimum of 1 year of nursing experience within the last 3 years.
- Minimum of 2 years of homecare experience.
- Minimum of 1 year of supervisory experience.
- Strong leadership, communication, and interpersonal skills.
- Excellent verbal and written communication skills.
- Current CPR certification.
- Valid driver's license and reliable automobile with insurance meeting Agency and state requirements.
- Ability to work independently, prioritize responsibilities, and effectively lead a clinical team.
Responsibilities
- Manage and provide clinical direction to a team of RNs, LPNs/LVNs, and Home Health Aides.
- Ensure clients receive safe, effective, and appropriate homecare services.
- Review referrals, determine required services, and assign appropriate clinicians to cases.
- Provide ongoing guidance, support, and direction to clinical staff.
- Assist clinicians with establishing priorities, setting goals, and evaluating client progress.
- Attend case conferences and clinical meetings to coordinate client care.
- Participate in the clinical on-call rotation as requested.
- Complete client care visits and shifts as directed by the Director of Clinical Services.
- Provide clinical support and education to Agency staff, clients, families, and healthcare professionals.
- Monitor clinical documentation for accuracy, completeness, and timeliness.
- Complete quarterly client chart reviews and additional reviews as needed.
- Assist with interviewing, hiring, orientation, and training of new clinical personnel.
- Conduct timely annual performance evaluations for clinical staff.
- Perform client admissions, supervisory visits, and reassessments as directed.
- Develop and update individualized client care plans as appropriate.
- Manage medical practitioner orders for client care and treatment.
- Assist with marketing and public relations activities as directed.
- Perform other duties as assigned.
