Registered Nurse – Outpatient Primary Care
We are seeking an experienced Registered Nurse to join a mission-driven outpatient primary care team in San Jose, California. This full-time position is ideal for an RN with at least one year of experience in family practice, primary care, or a clinic setting who enjoys care coordination, patient education, triage, and working within an integrated care team.
The Registered Nurse will support physicians with patient care, develop care plans, conduct monthly telehealth visits with Enhanced Care Management patients, review laboratory and diagnostic results, triage calls and walk-ins, coordinate specialty referrals, and provide direct nursing care within scope of practice. The RN may also provide clinical leadership to Medical Assistants and other ancillary staff when needed.
Position Highlights
- The salary range is $115,000 to $135,000 per year.
- The schedule is Monday through Friday from 8:30 a.m. to 5:30 p.m.
- This is a full-time, outpatient position with no weekend or overnight shifts.
- The position supports two San Jose clinic locations, with employees typically assigned to one clinic per day.
- Candidates must have at least one year of RN experience in family practice, primary care, or an outpatient clinic.
