Job Summary
We are seeking a dedicated and compassionate Case Manager to join our dynamic team. In this vital role, you will coordinate client care plans, facilitate communication among multidisciplinary teams, and ensure seamless transitions across various settings. Responsibilities
- Conduct comprehensive patient assessments.
- Develop personalized care plans that align with appropriate guidelines, waiver services and client needs.
- Collaborate with providers and community resources to ensure comprehensive client support.
- Perform utilization review and management activities to optimize care delivery and ensure compliance with regulatory standards such as HIPAA and HCBS Waiver services compliance.
- Maintain accurate documentation within Electronic Medical Record (EMR) or Electronic Health Record (EHR) systems, including detailed case notes and care plans.
- Communicate effectively with patients and their families.
- Travel to client homes on consistent basis to complete appropriate assessments.
Skills
- Proficiency in EMR/EHR systems and Microsoft Office suite.
- Excellent client communication skills with the ability to work effectively with individuals from diverse cultural backgrounds.
- Knowledge of HCBS Waivers (CES, SLS, DD and CHRP), as well as CFC Services (Homemaker and Personal Care)
- Bilingual abilities are a plus to enhance patient engagement across diverse populations.
