Every day, nurses coordinate care, support families, mentor caregivers, and make the decisions that keep patients safe. Yet most home care agencies leave nurses with little autonomy, limited financial upside, and no path to ownership.
We're building a different model. Hearth is a platform that helps experienced nurses start and grow their own home care practices. You own the patient relationships and lead the clinical care. We provide the business infrastructure - including standing up your practice, scheduling, billing, payroll, technology, compliance, and operations support- so you can focus on building a great practice instead of running a back office.
We're looking for our first Founding Nurse Partners to help launch this new model.
What You'll Do
Your primary role is to build and grow your own home care practice with Hearth's support:
- Build relationships with patients and families in your community
- Conduct assessments and develop individualized care plans
- Lead ongoing care and serve as the primary clinical point of contact for patients and families
- Build, train, and clinically supervise your team of caregivers
- Deliver high-quality, relationship-based care while growing your own practice
Unlike running a traditional home care agency on your own, we will support you in sourcing caregivers, managing payroll, billing, and handling administrative work so you can focus on leading care.
Help Shape the Future of Hearth
As one of our Founding Nurse Partners, you'll also help us build the platform.
You'll meet regularly with our team to share feedback on workflows, technology, and operations, helping shape the tools and support future nurse owners will use.
This is a small but meaningful part of the role - your experience will directly influence how we build Hearth.
Who We're Looking For
We're looking for experienced nurses who have thought about building something of their own. You don't need previous business experience - we're here to help with that.
You might be a great fit if you:
- Hold an active nursing license (NPs and experienced LPNs are also encouraged to apply)
- Have experience in home health, home care, hospice, care management, or a related setting
- Enjoy building relationships with patients and mentoring caregivers
- Want more autonomy and ownership over your work
- Are excited about helping build a new model of home care from the ground up

As your practice grows, so does your earning potential. We believe mature practices on the Hearth platform can generate $200,000+ in annual take-home income for nurse owners while giving you significantly more autonomy than a traditional agency role.
