The Opportunity
We're hiring a Hospice Community Liaison in Houston to lead referral development and community outreach for our growing hospice service line. This is a role for someone who's built referral networks in hospice and wants to do that work at a company that is genuinely investing in its people, its clinical standards, and its market presence.
You'll be the face of American Health at Home to referring physicians, hospital discharge planners, skilled nursing facilities, senior living communities, and other referral sources across the Houston market. You'll build the relationships, generate the qualified referrals, and represent the quality of our hospice care at every touchpoint.

What You'll Do
- Own referral development in the Houston market. Build and maintain a network of referring providers — physicians, hospitals, SNFs, senior living communities, home health agencies, and other community organizations that serve hospice-eligible patients
- Consistently meet and exceed admissions and referral targets. Track your own performance against clear KPIs; partner with our clinical operations team to convert referrals into admissions
- Represent American Health at Home in the community. Conduct educational in-services, community outreach events, and family education sessions that reflect the quality of our clinical care
- Educate patients and families on hospice care. Meet with prospective patients and their families to explain what hospice provides, address concerns, and support them through the transition
- Coordinate care transitions. Work closely with our clinical operations team to ensure smooth handoffs from acute care settings, physician offices, and other referral sources into hospice services
- Diversify referral channels. Identify new referral sources, develop new partnerships, and expand our presence across the Houston market
- Uphold our operational and compliance standards. Ensure all referral development activity meets regulatory requirements and reflects our commitment to clinical excellence
What You Bring
- 2+ years of hospice sales, marketing, or referral development experience (required); 5+ years preferred
- Proven track record of meeting and exceeding referral and admissions targets in a hospice or post-acute setting
- Deep understanding of the hospice referral ecosystem — physicians, hospitals, SNFs, senior living, home health, and community organizations serving seriously ill patients
- Strong interpersonal skills — you build trust quickly with clinicians, administrators, patients, and families
- Self-directed work style — you manage your own territory, prioritize your time, and hit your numbers
- Local Houston market knowledge and relationships are strongly preferred
- Reliable transportation and willingness to travel throughout the Houston metropolitan area
- Valid driver's license and clean driving record
