Job Overview
Spirit Homecare & Hospice is seeking a licensed Physical Therapist (PT) to provide home health physical therapy services throughout Clermont County, OH. This PRN position offers flexible scheduling, competitive per-visit compensation, and the opportunity to make a meaningful impact by helping patients regain mobility and independence in the comfort of their own homes.
As a Home Health Physical Therapist, you will evaluate patients, develop individualized treatment plans, provide skilled therapy services, and collaborate closely with an interdisciplinary care team to deliver exceptional patient care.
Compensation & Benefits
- Start of Care / Resumption Visits: $100 per visit
- Routine Visits: $55 per visit
- Flexible PRN Schedule – Accept as many or as few visits as fit your availability.
- Evening, weekend, and holiday coverage opportunities may be available.
Key Responsibilities
- Perform comprehensive in-home physical therapy evaluations and reassessments.
- Develop and implement individualized treatment plans based on each patient's goals and clinical needs.
- Provide therapeutic interventions focused on improving mobility, strength, balance, endurance, and overall functional independence.
- Educate patients and caregivers on home exercise programs, fall prevention, safe mobility techniques, and equipment use.
- Collaborate with Physicians, Nurses, Physical Therapy Assistants (PTAs), and other interdisciplinary team members to coordinate patient care.
- Complete clinical documentation accurately and in a timely manner.
- Provide clinical supervision and guidance to Physical Therapy Assistants (PTAs) when appropriate.
Qualifications
- Active Ohio Physical Therapist (PT) license (Required).
- Home Health experience preferred; Skilled Nursing Facility (SNF) or Outpatient experience will also be considered.
- Familiarity with Medicare home health documentation is a plus.
- Strong clinical judgment, communication, and organizational skills.
- Ability to work independently in a field-based environment.
- Valid driver's license, reliable transportation, and current auto insurance.
Service Area & Work Conditions
- Coverage Area: Clermont County, OH and surrounding communities.
- Setting: Skilled physical therapy services provided directly in patients' homes.
- Physical Requirements: Position may require standing, walking, bending, lifting, and assisting patients during treatment activities.
