Responsibilities:
- Provide skilled nursing care under the direction of a Registered Nurse
- Administer medications and treatments as prescribed
- Monitor patient status and report changes
- Educate patients and caregivers
- Document visits accurately and timely using EMR
- Collaborate with the care team to ensure optimal outcomes
Requirements:
- Active LPN license in Virginia
- 1+ year of clinical experience (home health a plus)
- Strong communication and documentation skills
- Reliable transportation and valid driver’s license
- Self-motivated and able to work independently
