Responsibilities
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Monitor patients’ health status by checking vital signs such as blood pressure, pulse, and temperature; report any concerns promptly
- Administer medications accurately following care plans and HIPAA confidentiality standards
- Support patients with meal preparation and nutritional needs, ensuring dietary guidelines are followed
- Utilize specialized equipment such as Hoyer lifts or resident’s <PERSON> to safely transfer individuals with mobility challenges
- Maintain detailed documentation of patient observations, care provided, and any changes in condition to ensure continuity of care
- Perform light housekeeping duties including cleaning living areas to promote hygiene and comfort
- Provide companionship and emotional support to reduce feelings of loneliness or anxiety, especially for those with dementia or Alzheimer’s disease
Requirements
- Proven experience in home & community care or assisted living environments; group home experience is highly valued
- Knowledge of patient observation techniques and ability to recognize signs requiring medical attention
- Certifications such as CPR, First Aid, and HIPAA compliance are required; additional certifications in dementia care or hospice are a plus
- Ability to lift heavy objects safely and perform physical tasks necessary for patient transfer and mobility assistance
- Strong communication skills to collaborate effectively with clients, families, healthcare professionals, and social workers
- Experience working with individuals with developmental disabilities, autism, or memory care needs is preferred
- Valid driver’s license and reliable transportation may be required for travel between client homes
