The Home Care Intake Specialist serves as the first point of contact for patients, families, referral sources, and payers seeking home care services. This role is responsible for managing the end-to-end intake process — from initial referral through authorization and case setup — while ensuring compliance with New York State Department of Health regulations, Medicaid/managed care requirements, and agency policies. The ideal candidate is highly organized, detail-oriented, and skilled at coordinating across clinical, administrative, and insurance teams.
Key Responsibilities
Referral & Intake Processing

Receive and process referrals from hospitals, physicians, discharge planners, managed care organizations (MCOs), and self-referrals

Collect and review all necessary intake documentation, including physician orders, medical records, insurance information, and demographic data

Enter patient information accurately into the agency's Electronic Health Record (EHR) / intake system

Determine patient eligibility for home care services based on payer requirements and NYS DOH criteria

Screen referrals for appropriate level of care: CHHA, LHCSA, or private duty nursing

Communicate intake status updates to referral sources, families, and clinical supervisors in a timely manner
Qualifications
Required

High school diploma or GED required; Associate's or Bachelor's degree in Health Administration, Social Work, Nursing, or related field preferred

Minimum 1-2 years of experience in home care intake, hospital discharge planning, or healthcare admissions

Working knowledge of NYS Medicaid, MLTC managed care plans, Medicare, and private insurance

Familiarity with home care service types: CHHA, LHCSA, and Private Duty Nursing

Proficiency with EHR/intake software and Microsoft Office Suite (Word, Excel, Outlook)

Excellent verbal and written communication skills

Strong organizational skills and ability to manage multiple referrals simultaneously in a fast-paced environment
