The Licensed Clinical Social Worker-Associate (LCSW-A) provides integrated behavioral health services as part of a multidisciplinary care team. The LCSW-A offers assessments, brief interventions, counseling, and referrals to help patients manage emotional, behavioral, and psychosocial challenges that impact their overall health. The LCSW-A will also provide assistance to the care management team by working collaboratively to support patient care plans and needs.
Key Responsibilities
- Provide evidence-based short-term counseling and crisis intervention to patients across the lifespan.
- Conduct psychosocial assessments and develop individualized care plans.
- Collaborate with primary care providers and other clinical team members to support patient treatment goals.
- Engages with patients to identify and address barriers that impede health outcomes
- Facilitate referrals to specialty mental health and community resources as needed.
- Maintain accurate and timely clinical documentation in the electronic health record (EHR) and the care management software.
- Participate in care team meetings, case reviews, and quality improvement initiatives.
