About the Role: We are seeking a dedicated and compassionate Family Medicine Physician to provide high-quality care in post-acute care settings. This role focuses on managing patients transitioning from hospital to skilled nursing facilities, rehabilitation centers, or home-based care. The ideal candidate is comfortable working with medically complex patients, coordinating multidisciplinary care, and improving patient outcomes during recovery.
Key Responsibilities:
- Provide comprehensive medical care to patients in post-acute settings, including skilled nursing facilities and rehabilitation centers
- Conduct initial assessments, routine follow-ups, and acute visits as needed
- Manage chronic conditions, medication reconciliation, and care plans following hospital discharge
- Collaborate with nurses, therapists, case managers, and specialists to ensure continuity of care
- Communicate effectively with patients and families regarding treatment plans and goals of care
- Participate in quality improvement initiatives to reduce readmissions and enhance patient outcomes
- Maintain accurate and timely documentation in the electronic health record (EHR)
Pay: $250,000.00 - $280,000.00 per year
