What you'll do as a Independent Living Case Manager:
- Work Monday through Friday 8am-4pm
- Provide support to 8-9 individuals both in their home and within the community.
- Utilize your personal vehicle to offer transportation services.
- Provide role modeling, training and direct care services encompassing all aspects of daily living
- Aid individuals in managing state issued benefits and government programs.
- Foster participation in individuals’ social and recreational activities tailored to individual interests.
- Assist in completing the ISP process for DDS funded individuals.
- Respond to the emergency needs of individuals.
- Oversee financial budgets, balance check books, bank deposit, timely payment of bills of persons on assigned caseload.
What you'll need to be a Independent Living Case Manager:
- High School Diploma or General Education Degree (GED) required
- Valid driver's license, access to a motor vehicle, and satisfactory driving record
- Strong written and verbal communication skills
- Strong computer and word processing skills
- 1+ years’ experience and/or training working with individuals with disabilities preferred.
- 1+ years’ experience in case management preferred.
