MDS Coordinator/RNAC/LNAC
Job Summary
We are seeking a highly skilled and experienced MDS Coordinator/RNAC/LNAC to join our team at Stroudsburg Post Acute Nursing and Rehabilitation in Stroudsburg, PA. As a key member of our healthcare team, you will be responsible for ensuring the accurate and timely completion of all Medicare/Medicaid case-mix documents to ensure appropriate reimbursement for care and services provided within the Facility. Your expertise will also be utilized to continually review Minimum Data Set (MDS) assessments to achieve optimal Resource Utilization Group (RUG) categories.
Essential Duties and Responsibilities
Assess and determine the health status and level of care of all new admissions
Ensure the accurate and timely completion of all MDS Assessments, including PPS Medicare, quarterly, annual, and significant change assessments
Communicate level of care for new residents to all disciplines
Coordinate interdisciplinary participation in completing the Minimum Data Set (MDS) for each new admission to the facility
Maintain an accurate schedule of all MDS assessments and ensure completeness and thoroughness of documentation as mandated by federal, state, and medical standards
Responsible for data entry and electronic submission of MDS assessments, verifying electronic submissions and performing corrections when necessary
Coordinate interdisciplinary participation in completing the MDS for each resident according to regulatory time frames
Schedule and conduct resident care conferences in compliance with state and federal regulations
Assist disciplines in formulating and revising care plans, ensuring that resident's present/potential problems are identified and prioritized
Evaluate resident care plans for comprehensiveness and individuality, assessing the achievement or lack of achievement of desired outcomes
Responsible for all level of care changes within the facility, notifying all departments of level of care changes
Generate appropriate forms to complete level of acuity and changes, transmitting forms to the appropriate agency for processing as required by state law
Qualifications
RN or LPN with an unencumbered PA state licensure
Long Term Care Experience preferred
Ability to read, write, speak, and understand the English language
Physical Demands
Required to sit, stand, bend, and walk regularly
Lift and/or move up to 25 pounds
Visual and auditory ability sufficient for written and verbal communication
The noise level in the work environment is usually moderate
