Job Summary:
The Nurse Care Manager is responsible for delivering professional nursing care to our members, supervising caregivers, and ensuring compliance with all AFC regulations. This role involves frequent travel for in-home visits, documentation, and collaboration with the multidisciplinary team to maintain high-quality care standards.
Key Responsibilities: Clinical Oversight:
- Provide and support care service needs.
- Develop, review, and update care plans, ensuring alignment with member needs and AFC regulations.
- Conduct on-site visits.
- Address member concerns and needs with the Multidisciplinary Team.
Caregiver Support:
- Provide support, education, and resources on ongoing basis.
- Monitor caregiver compliance with AFC requirements and provide ongoing support.
Documentation and Reporting:
- Conduct monthly visit documentation.
- Complete Care Plans.
- Completion of semi-annual and annual health status reports and communicate findings with primary care physicians.
- Prepare and complete Minimum Data Set (MDS) documentation, including pre-visit preparation, in-home review, and post-visit submission.
Team Collaboration:
- Work closely with Care Managers and MDT to support Member and Caregiver needs.
- Attend Department and Organization meetings and training sessions.
Qualifications:
- Current and unrestricted Nurse license in the state of Massachusetts.
- Two years experience in adult care preferred.
- Certified in CPR and First Aid.
- Strong organizational and communication skills.
- Reliable transportation.
- CORI background check, physical exam within the past 12 months, and an annual TB screening are required.
Working Conditions:
- Hybrid role in Massachusetts, with travel for home visits.
- Periodic in office meetings and trainings required.
- Use of office equipment such as computers and fax machines for documentation and reporting as well as use of Microsoft Suite and EMR.
