The Licensed Practical Nurse (LPN)/ Referral Management Center Liaison is responsible for the timely processing of inquiry and referral calls. Serves as liaison to referral sources, physicians, unit staff and hospital personnel involved with each transfer request. Enters call data into the facility’s database and manages all e-fax application(s) to process incoming referrals and transfer requests in a timely manner. Effectively communicate with unit staff, physicians, and Admissions Triage Staff. Follow hospital’s policies and procedures regarding referral management. This is a PRN weekend position.
Graduate of an accredited Licensed Practical Nurse (LPN) program. Prefer one (1) year experience as an LPN. Prefer six (6) months in psychiatry nursing. Knowledge of current nursing procedures, the nursing process, medications and their correct administration.
