Title of Position: Case Manager
Department: Programs
Reports to: Senior Community Programs Manager
Position Summary
The ProjectRISE Case Manager provides individualized support and case management services to current ProjectRISE students and program alumni. This role helps participants identify and achieve educational, career, financial, and personal goals through coaching, resource coordination, advocacy, and ongoing engagement.
The Case Manager also plays an active role in supporting ProjectRISE cohorts by participating in classes, workshops, and program activities as assigned. This position requires the ability to safely operate The Sharing Center passenger van to transport participants when needed.
Essential Responsibilities
Case Management
- Maintain an active caseload of current students and alumni.
- Conduct assessments and develop individualized success plans.
- Provide ongoing coaching, support, accountability, and follow-up.
- Assist participants in navigating barriers and accessing resources.
- Connect participants with internal programs and community services.
- Monitor participant progress and document outcomes.
Alumni Engagement
- Maintain regular contact with program alumni.
- Provide continued support, referrals, and goal-focused coaching.
- Track alumni outcomes and program engagement.
- Encourage participation in mentoring and networking opportunities.
Cohort Support
- Participate in ProjectRISE classes, workshops, and cohort activities as assigned.
- Support instructors and facilitators during classroom sessions.
- Foster participant engagement and a positive learning environment.
- Facilitate discussions or activities when requested.
Transportation
- Safely operate The Sharing Center passenger van to transport participants as needed.
- Provide transportation to classes, program activities, appointments, and special events.
- Follow all transportation safety policies and procedures.
- Report vehicle maintenance or safety concerns promptly.
Documentation & Collaboration
- Maintain accurate participant records, case notes, and program documentation.
- Ensure compliance with grant, program, and organizational requirements.
- Enter and update information in designated databases and reporting systems.
- Collaborate with staff, volunteers, instructors, and community partners.
- Represent ProjectRISE at meetings and community events as assigned.
- Perform other duties as assigned.
Qualifications
Required
- Passion for The Sharing Center's mission and commitment to participant success.
- Bachelor's degree in Social Work, Human Services, Education, Counseling, Psychology, or a related field; equivalent experience may be considered.
- Experience in case management, coaching, education, workforce development, or related services.
- Strong interpersonal, communication, and organizational skills.
- Ability to work effectively with individuals from diverse backgrounds.
- Proficiency with Microsoft Office and database systems.
- Valid Florida driver's license with an acceptable driving record.
- Ability and willingness to drive a passenger van.
Preferred
- Experience working with students, young adults, or workforce development programs.
- Knowledge of community resources and support services.
- Bilingual English/Spanish.
Physical Requirements
- Ability to sit, stand, walk, and travel locally as needed.
- Ability to occasionally lift up to 25 pounds.
- Ability to safely operate a passenger vehicle.
Schedule
Monday through Friday during normal business hours. Occasional evenings, weekends, and special events may be required.
Salary Range: $40,000-$45,000

Some standing is needed, but most of the time, it will be in an office environment.
