We are currently seeking a compassionate and skilled Home Health Physical Therapist Assistant – PTA to provide personalized, one-on-one therapy services in a home setting throughout Danville and the surrounding coverage areas. This position is ideal for a PTA who enjoys working independently while helping patients regain strength, mobility, balance, and overall functionality in their daily environment.
As a Home Health Physical Therapist Assistant – PTA, you will work under the supervision of a Physical Therapist to carry out individualized treatment plans for patients recovering from illness, injury, surgery, or chronic conditions. This role requires travel to patient homes throughout the service area's.
- Provide home-based physical therapy services under the direction of a supervising Physical Therapist
- Assist patients with therapeutic exercises, stretching, balance training, gait training, and mobility activities
- Monitor patient progress and report changes in condition to the supervising Physical Therapist
- Reinforce and carry out individualized treatment plans established by the Physical Therapist
- Educate patients and family members on exercises, mobility techniques, safety precautions, and home exercise programs
- Assist patients in improving strength, endurance, coordination, and functional mobility
- Document patient visits, progress, and treatment responses accurately and timely
- Collaborate with interdisciplinary team members including RNs, LPNs, CNAs, OTs, and SLPs to support patient goals and continuity of care
- Maintain compliance with home health regulations and company policies
- Graduate of an accredited Physical Therapist Assistant (PTA) program
- Active PTA license in the state of Virginia
- Minimum of 1 year of PTA experience preferred (home health experience preferred)
- CPR certification and TB baseline screening required
- Strong communication and interpersonal skills
- Good clinical judgment and ability to work independently
