LPN Triage Nurse
Reports To: RHC Clinical Supervisor
JOB DESCRIPTION SUMMARY
The LPN Triage Nurse will process incoming calls from families, patients and others. The triage nurse will work cooperatively with physicians and the hospice team of multi-disciplinary professionals to update the plan of care, follow prescribed medical treatments (including pain management and symptom control), and provide education and supportive care to patients
and caregivers.
ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES

Answer all incoming calls in a manner appropriate to the nature of the call, rapidly identify the caller's needs, and takes proper action to assure the caller's needs are handled.

Orders all DME for new admits and current patients.
- Assists in the implementation of improved work methods and procedures to ensure compliance with Federal and State laws and compliance with Company policies.

Builds and monitors community and customer perceptions of Hospice of Acadiana, Inc as a high-quality provider of services.

Prepare, coordinate and communicate the daily RN Case Manager schedules.
- Participates in quality assessment performance improvement teams and activities.

Collaborates with pharmacies, DME vendors, and other agencies for effective patient management. Facilitates delivery or maintenance of provided medical equipment to meet patient needs. Collaborates to order necessary testing and imaging.
- Submits accurate and detailed documentation in real-time to promote continuity of care in a timely manner.

Attends required meetings and educational sessions.

Assist with Volunteer Physician management.

POSITION QUALIFICATIONS
- Certifications/Licensure/Registration: Valid Louisiana Licensed Practical Nurse (LPN).

Education: High School Diploma or equivalent; Completion of an approved, accredited practical nursing program.

Experience: Minimum three (3) years’ experience as an LPN; Hospice experience preferred. Possesses valid Louisiana Driver’s License, Automobile Liability Coverage, BLS CPR cert.

ENVIRONMENTAL CONDITIONS: Employee may be exposed to extremes of heat and cold in all weather conditions. Employees may also be exposed to dust, fumes or gases. Must drive in various weather conditions on roads in varying degrees of repair.
WORKING CONDITIONS: Employee may be exposed to infections/contagious diseases. May have contact with patients under a variety of circumstances. May be near moving equipment. Handles emergency crisis situations. Travel required.
