American Traveler is hiring an RN Case Manager for a Hospice position requiring active CA RN licensure and one year of hospice experience.

Home health and hospice care setting
-
Serves patients throughout San Francisco, Brisbane, Daly City, Colma, South SF, Pacifica, and neighboring cities as needed
-
Day shift schedule from 8:30am-5pm with flexibility based on patient availability

One weekend per month required
-
13-week assignment

Active CA RN license required

BLS certification required
-
Minimum one year of hospice experience required

Admit patients to hospice services and assess physical, psychosocial, environmental, safety, and developmental needs of patients and families

Develop individualized plans of care based on comprehensive assessments

Document observations and interventions related to the patient's environment
-
Coordinate care with a multidisciplinary team
-
Participate in the hospice program's quality assessment performance improvement program
-
Conduct follow-up visits as needed
